How It Works:

2) Complete the online rebate form (Be sure to Print and Sign the online confirmation page)
3) To finalize the rebate submission you will need the following:

• Signed printout of completed online submission form confirmation page
• A copy of the purchase receipt
• Eligible trade-in pager units

4) Mail the above documentation/items to:

Motorola Minitor V Pagers cannot be returned once the original rebate submission form has been submitted. Not responsible for lost, late, mutilated, misdirected or postage due mail. Illegible or incomplete forms will be considered invalid and ineligible for offer. This offer may not be combined with any other promotional offers, rebates, coupons or discounts.
